How It Works
The Target
Cardiolipin — a phospholipid found exclusively in the inner mitochondrial membrane. SS-31 concentrates 1,000-fold inside mitochondria due to its positive charge.
The Mechanism
Binds to cardiolipin and stabilizes the electron transport chain complexes (particularly Complex III and IV), preventing electron leak that generates damaging reactive oxygen species. It also preserves the organization of respiratory supercomplexes — the physical arrangement of the electron transport chain that determines mitochondrial efficiency.
Why It Matters
SS-31 (Elamipretide) is in clinical trials for heart failure and Barth syndrome. It represents a new therapeutic approach: rather than scavenging ROS after they form, it prevents their generation at the source by optimizing mitochondrial structure.
